Details of nominated mentor

Gender (of nominated mentor)*
Date
Date the apprentice(s) that the mentor has been supporting is due to complete their training. DD can be an estimate as long as month and year are accurate.

Details of nominator. Please fill in this section even if nominating yourself.

Mentor nomination:

Mentor nominations are accepted for both workplace colleagues and staff who offer apprentices support from a training provider. Judges will be scoring nominations based on the criteria below. Please complete each section, describing what your nominee has done to achieve each criterion and why they are worthy winners of the award.
Judges will be looking for evidence and practical examples that the mentor has supported the apprentice to develop significantly in their job role, enjoy increased levels of responsibility and achieve academically. Top marks will not only be given to those supporting the highest achievers, but those who have supported learners to achieve significantly by their own standards. To score top marks there should be evidence of tangible examples where the mentor has gone above and beyond to offer exceptional support.
Judges will be looking for evidence and examples of when that the mentor has made significant efforts to support the apprentice to mature and develop confidence both professionally and personally. For top scores there should be tangible examples of the mentor going above and beyond to encourage this development and to offer their support.
Judges will be looking for reasons that set this nomination apart and make it stand out as a potential winner. For a top score, the mentor should be an example for others to follow. Judges will be looking for practical examples of support provided and will consider and rate the mentor's support based on the value it would add to their own organisation and its less experienced staff.

If you wish to send examples of your nominee's work to support your nomination, please email them to nominations@truro-penwith.ac.uk with your nominee's name as the subject. This evidence should not be used to extend the 500 words of your nomination. Truro and Penwith College may use content from your nomination in press coverage of the Cornwall Apprenticeship Awards and to highlight outstanding achievement in Apprenticeship training in Cornwall. Footage and photographs taken on the evening will also be used. If there is any reason why a nominee or their achievements should not be published in the media please get in contact with us at nominations@truro-penwith.ac.uk

Judges will be looking for a combination of the employer supporting both academic achievement and personal development. To achieve a top score, learners will have been supported in their studies to achieve the best possible outcome by their own personal standards. There should also be strong evidence and practical examples of learners being given good levels of responsibility, opportunities to develop their skills, experience and to develop on a personal level.
In this section judges will look for reasons that set this nomination apart and make it stand out as a potential winner. For a top score, the programme, and practical examples of its success, should be an example for other employers to follow, judges will consider and rate the apprenticeship programme's outcomes based on the value they would add to their own business and its teams.

Apprentice nomination

Judges will be scoring nominations based on the criteria below. Please complete each section, describing what your nominee has done to achieve each criterion and why they are worthy winners of the award.

Nominees should be entered into the most appropriate sector category and will be automatically considered for Cornwall's Apprentice of the Year and Achiever of the Year awards. If the sector categories are not appropriate please nominate for Cornwall's Apprentice of the Year.
Judges will be looking for evidence that contributions or ideas have been adopted and implemented as a permanent solution by the employer. To achieve a top score there should be evidence of tangible benefits i.e. improved efficiency, cost savings, increased sales, added value etc.
Judges will be looking for a combination of academic achievement and academic improvement throughout the apprenticeship. Top scores will not only be awarded to nominees with the best grades, but those who have overachieved by their own standards, from where they were at the start of their apprenticeship programme.
This area considers skills and qualities that the apprentice has brought to their role in addition to the basic requirements of their employer. Those achieving top scores will have gone above and beyond the call of duty to evidence great team working or commitment and may be mentoring others or helping the organisation to promote their apprenticeship programme.
Judges will score the value the apprentice has added to their team, organisation or community. If this apprentice has overome adversity or personal challenge, tell us about it here so we can also consider them for the Achiever of the Year Award.

If you wish to send examples of your nominee's work to support your nomination, please email them to nominations@truro-penwith.ac.uk with your nominee's name as the subject.
